What's included in the Short Form Certificate?
The Short Form is a quick, official confirmation that a business entity exists and is in good standing with New York State.
- Legal name of the entity
- Entity type (corporation, LLC, etc.)
- State of formation or registration
- Current status (active or inactive)
- Confirmation of compliance with biennial filing requirements
Not included: officers, directors, members, registered agent, or filing history.
What's included in the Long Form Certificate?
The Long Form is a complete, detailed record of the entity — everything in the Short Form, plus:
- Current officers, directors, or members
- Registered agent and office address
- Full filing history (articles of incorporation, amendments, and more)

Which one should you order?
Since the Long Form costs the same as the Short Form in New York, most title companies, lenders, and attorneys request the Long Form by default — it gives a complete record with no added cost. The Short Form can still be the right choice when a requesting party only needs basic confirmation of active status.
